To apply to the Radiography Online Degree Completion program, please download the materials below. Be sure to read and follow all the directions on the General Information page. A checklist has been provided for your convenience. Application materials are the applicant's responsibility to completely fill out and turn in correctly. Adobe Acrobat is required to open some of the following documents. If you do not currently have Adobe Acrobat Reader, please click here to download and install Adobe Acrobat Reader.

You may download the application in Microsoft Word or Adobe PDF. The Microsoft Word will allow you to fill out the application on your computer. The PDF will allow you to print the application to fill out by hand. Please be sure to follow all instructions and mail the application back to us with your signature.

Once you have filled downloaded the application, you may download the supporting application materials that will need to be turned in along with, or within 2 weeks of, the application.

	return a completed and signed application to: Radiography Degree Completion Admissions Committee School of Health Professions University of Missouri-Columbia 605 Lewis Hall Columbia, MO 65211-4230 (Fax: 573-884-1490)
	forward transcripts from every school attended along with ACT scores
	send official transcripts of completed college work from every school attended to the above address (a photostatic or unofficial copy of your transcript for application will suffice until the official transcript is received)
	Ask three (3) people to provide an evaluation (must be sent directly to Radiography Applications Committee, not to you)
