Application Forms for the Occupational Therapy Professional Program
- Available October 1 through December 31
- Form links will be activated only during application period
- Submit forms together to the MUOT Department postmarked by December 31st
- Instructions are included with each form

The "OT Observation Form 
" is available year round. 30 hours of OT observation is required and must be recorded and signed by a registered Occupational Therapist on the Observation Form provided. Any additional hours should be reported on the Activities Form.

- Intent to Apply : Fill out and mail immediately
- SHarP Scholar Intent : For SHarP Scholars ONLY
- Application : This is the main application form
- Observation Form: Should accompany each OT observation
- Reference Form: Should accompany each of your 3 references
- Activities Form: Volunteer, work and leadership experiences
- Personal Statement Form: Short bio explaining how you came to choose OT as a career
- Request for Transcript Evaluation: Sign and include with packet for transcript(s) evaluation
Point Values for Application:
- 50% GPA
- 18% Activities (13% OT Volunteer/Work and 5% Leadership/Awards)
- 5% Personal Statement
- 5% References
- 20% Interview
- 2% MU Student
