Application
All students interested in attending the Professional OT Program at MU must follow application instructions below and are required to apply and be accepted before they can begin classes.
Application Forms for the Occupational Therapy Professional Program
- Available October 1 through December 31 (except Observation/Volunteer Forms below)
- Form links will be activated only during application period
- Submit forms together to the MUOT Department postmarked by December 31st
- Instructions are included with each form

OT Observations and Volunteer Hours
The "OT Observation Form
" and "Volunteer Form"is available year round.The following hours are a basic requirement for application and must be completed by application deadline.
- 30 hours of OT observation is required and must be recorded and signed by a registered Occupational Therapist on the Observation Form provided.
- 20 hours of volunteering is required and must be recorded and signed by volunteer coordinator or supervisor on Volunteer Form provided.

- 50% GPA
- 15% Personal Statement
- 5% References
- 15% Interview
- 10% Interview writing sample
