Early Admission - Application

GENERAL INFORMATION FOR APPLICATION
DOCTOR OF PHYSICAL THERAPY

Application materials must be received by the MU PT Department by January 24. GRE scores will be mandatory for all applications.

NOTE: TO GET ALL THE NECESSARY INFORMATION AND FORMS, VISIT EACH LINK BELOW and PRINT THE MATERIALS:

3. Application Form -- [can be completed in 2 ways]

a. Word document - File / Save As. Then use the Tab key to move through the grey-shaded fields, while typing in the information. Print it. Sign it.

OR

b. Adobe pdf file - Print it. Complete it by using a typewriter, or by hand. Sign it.

4. Confidential Evaluation of Applicant (Recommendations) (print two copies)

DO NOT COMPLETE THE APPLICATION UNTIL YOUR FALL SEMESTER FINAL GRADES HAVE BEEN RECEIVED AND ARE INCLUDED ON THE ENCLOSED FORMS.

Make a photocopy of your completed application for your future reference.

The items below should then be submitted to the University of Missouri Physical Therapy program at:

Transcripts requested from all schools attended (fall grades included) and received by Physical Therapy Department by January 24th
**Students enrolled at MU do not need to submit transcripts unless current MU transcript does not contain all college coursework completed.

Non-University of Missouri-Columbia students will not be required to apply for general admission to the University of Missouri-Columbia until they have received notification of their acceptance (or non-acceptance) to the Physical Therapy Program.
Applicants who wish to be eligible for financial aid must, however, apply to the University of Missouri Admissions Office by March 1.
